Job Description

Summary: The Infusion Hospital Liaison provides clinical education and marketing services to hospital and physician referral sources to ensure awareness of Preferred Homecare’s programs and services. Assesses and coordinates home infusion nursing services with appropriate candidates to assist with determining the plan of care administered by Preferred Homecare under the direction of the patient’s physician. Consults with physicians, pharmacists and other health care personnel to determine the best clinical course of treatment for the patient.
Essential Duties and Responsibilities include the following.  Other duties may be assigned. 
  • Maintains and grows core therapy admits through customer relationships and provides responsive customer service with assigned accounts. Evaluates current and future clinical and service needs for existing and new accounts. 
  • Provide patient and caregiver training on therapy, monitoring, and side effects. Demonstrate procedure for administering therapy in the home, with a return demonstration from the patient/caregiver. 
  • Demonstrates thorough knowledge of core therapy referrals, qualifying criteria both by payor source and branch. 
  • Establish and maintain clinical documentation required to track patient teaching provided and any other clinical documentation requested by Preferred Homecare
  • Collaborates with Case Managers, Payers, referral sources, home health agencies and Preferred Homecare staff to facilitate patient discharge and coordination of care.
  • Recognizes opportunities for expanding the business relationships with existing accounts and involves the Territory Manager, Regional Director of Sales, or the VP of Sales when appropriate to present Preferred Homecare’s other programs or negotiate terms of service.
  • Provides clinical expertise in support of the sales effort by utilizing clinical skills to assist referral sources with identification of appropriate home infusion candidates, provides assessment of patients’ ability to perform care independently or with the assistance of an appropriate caregiver, provides patient teaching regarding therapy, supplies, and safety. Provide enteral/infusion pump teaches or other patient care needs in order to facilitate discharge from a facility. 
  • Appropriately documents patient care activity, coordination of home infusion nursing services provided and all communication with patients, physician and referral source. 
  • Ensures all documentation is legible, accurate, complete, and complies with company policy and procedure for electronic and paper patient clinical record entries. 
  • Communicates/coordinates discharge planning activities for new patient referrals with branch personnel (Admissions, Clinical, Operations, and Reimbursement and Referral sources). 
  • Answers incoming telephone calls and triages to appropriate person as indicated. 
  • Participates in staff meetings with nursing and pharmacy. 
  • Participates in departmental QA activities as requested. 
  • Participates in cross training of the various clinical functions in order to function in multiple roles as required. 
  • Attends designated departmental and branch meetings as scheduled. 
  • Performs other tasks or duties as requested.

Education/Experience: Bachelor's degree (B. A.) from four-year College or university; or one to two years related experience and/or training; or equivalent combination of education and experience
Computer Skills: To perform this job successfully, an individual should have knowledge of Word Processing software.
Certificates and Licenses: Must have an LVN, LPN or RD license in the state in which they reside/practice.
