Job Description

Summary: This position is primarily responsible for scheduling patient visits with appropriate nurse based on skill-set, geographic area and time of next dose.
Essential Duties and Responsibilities include the following.  Other duties may be assigned. 
  • Pages and calls nurse as applicable with information regarding changes in their daily schedule.
  • Obtains the weekly schedule from the nurses and availability and updates the scheduling program accordingly.
  • Works closely with the Clinical Coordinator in communications between the office staff, nurses, and pharmacists.
  • Evaluates over-flow of case managers and works with nursing staff and management to staff these patients.
  • Maintains communication log and coordinates after hours and weekend visits with triage nurse. Faxes over schedule and delivery logs.          
  • Documentation is completed and updated in computer system in a timely manner.
  • Participates in morning meetings and shares findings from after hour on-call report logs and other pertinent information.

Education/Experience: High School diploma or GED equivalency.  At least two years related experience is required. Must be proficient using the computer; detail oriented and has effective time management skills. May have CNA, CPhT or LPN license and certification (not mandatory).
Computer Skills: To perform this job successfully, an individual should have knowledge of Word Processing software.
